Exploring the interplay among chronic disease, chronic wounds and social determinants of health

Chronic disease and the complexity of chronic wound management will take center stage in this discussion of how impaired circulation, compromised immune function, nutritional impairments, and other underlying factors impact the caregiving process. We’ll also explore the impact of the social determinants of health and discuss interventions most likely to achieve patient engagement and successful behavior change for wound healing under a tailored patient-centered approach.
